Santa Barbara Cottage Hospital Medical Social Work

The Medical Social Worker (MSW) is responsible for performing care coordination, resource linkage, clinical interventions, program development, utilization review, bio-psycho-social assessments, and discharge planning for patients admitted to Cottage Health. The MSWs primary focus is on assisting psychosocially complex patients as well as medically complex patients through their hospital stays to help facilitate successful discharges. MSW activities will result in quality outcomes, optimal care/cost management of services and/or procedures, a high level of customer satisfaction, and contribution to an overall value-oriented experience of stakeholders and persons served.
